Once your appointment is booked we email you a link to complete the required new patient paperwork online. Do it at home, at your own pace, and your first visit starts on time and stays on treatment.
If you would rather work on paper, or the emailed link gives you trouble, download the new patient packet, print it, and bring it filled out. No printer? Just arrive 15 to 20 minutes early and we will hand you a copy at the front desk.

We Email the Link
After you book, the paperwork link goes to the email address you gave us. Everything we need from you is in it.
You Complete It Online
Fill it out from home before your appointment. If you would rather use paper, print the packet above and bring it with you instead.
You Arrive and Start
Show up at your appointment time with your insurance card and photo ID. If the paperwork is not done yet, come 15 to 20 minutes early instead.

Send us your details and we will call you back with times. No referral needed in Utah, and most insurances are accepted.
